As a member of my practice you will enjoy the following services;
24/7 availability by phone/text and email. You will talk directly and only to me, I do not use mid-level providers or medical assistants for this service.
Same day, or next day appointments. House calls and/or office visits.
Annual Comprehensive Physical Exam.
Sub-specialty/Diagnostic and Laboratory referrals as needed.
Fee Schedule:
Annual Retainer $1500/year or a monthly fee of $125/month. Additional family members $85/month.
-your insurance will not be billed for any of my services. Outside costs/procedures such as laboratory and diagnostic testing will be billed to your insurance from the point of service. i.e. lab, hospital, or sub-specialist office.
